Hi,

I commit another fix (for the time-stamp), actually I just fixed what I
broke myself before. 

Now any combination of max_bframes, bquant_ratio and flags etc. should
work. There should be never any artefacts! 

Please test! If you find anything strange, please tell me! 

For results: Except from being SLOW, I had some new results about B-frame
quality:

Paris clip: (old was: max_bframes=1, bquant_ratio=150 or 200 is better
than no bfraes). 
New is: max_bframes=2 bquant=150/200 is not superior to max_bframes=1,
about equal. Others are worse. 

Commedian clip (720x576): all combinantion of max_bframes=1,2,3
ratio=80,100,150,200,250 are _inferior_ to max_bframes=0,  

Foreman clip (176x144): bframes are always better than
no-bframes, 
bframes=X sorted by quality is 0 < 1 < 3 < 2
ratio almost doesn't matter (in sensible range 100,150,200,250)
