xvidcore ony deals with "raw" mpeg-4 visual bitstreams.
to playback the video in popular applications (quicktime,envivio,etc.),
you have to embed the bitstreams in either a container format:
.avi, .mp4, etc.

the mpeg4ip project (http://sourceforge.net/projects/mpeg4ip) has a
tool for converting raw mpe4 visual bistreams to .mp4.

yes.
there are existing tools and libs for encoding/manipulating container
formats: eg. transcode, mplayer/mencoder, virtualdub, etc.
