[XviD-devel] custom quant matrix glitch

i just removed an undocumented feature whereby custom-matrix encodes had a vol header written for every frame, even if the matrix hadn't changed from the previous vol.

i had to alter the return value of set_inter_matrix() and set_intra_matrix().  i couldn't think up a situation where the new behaviour will generate empty matrix values or anything else horrible, and new bitstreams play just fine in xvid/dx50.  ffdshow reboots my win2k box, so no more testing with that.
