[XviD-devel] api change: bquant_offset

hi,

koepi has added bquant_offset support. this gives us a quadratic
for calculating bvop quantiziers. importantly, thi changes the api.

the forumula is:

b_quant =
      (AVG(prev vop quant, future vop quant) * bquant_ratio +
      bquant_offset) / 100

ie.  bquant_ratio=150, bquant_offset=50 will give "q = 1.5*AVG + .5"
     though ive set the default to 150,100.

additional changes:
* the decoder patch i posted a few days ago
* vfw nolonger reports bframes a keyframes
* vfw is updated to reflect koepis mods (gmc/qpel gui controls)
* vfw 2pass changes by foxer (untested by myself; iam assured they're
    functional
